This is a disclosure required by the Federal Electronic Signatures in Global and National Commerce Act (E-Signature Act) in order for us to provide you with disclosures and make agreements with you on-line. Please read this disclosure carefully prior to selecting the "I have read and accept" checkbox below.

You agree to the acceptance of loan documents, notices, periodic statements, and disclosures by means of electronic delivery.

You understand and agree that you have a right to receive any required notices, periodic statement, and disclosures in paper form as well as your right to withdraw your consent to have them delivered electronically.

You further understand and agree that in order to withdraw such consent you must provide Systems Services, Inc. with notice either in writing at P.O. Box 1559, Sikeston, Missouri 63801 or via e-mail at
SSIPR@systems-services-inc.com.

Once you have provided your consent to receive loan documents, notices, periodic statements, and disclosures electronically, you can still obtain paper copies of them by requesting such copies in writing or via e-mail as described above.

In order to access “http://www.systemsservicespayrolladvance.com” and to receive required loan documents, notices, periodic statements and disclosures electronically, you will need Internet access and either a Microsoft Explorer version 5 or higher, Firefox 2.0 or higher, Safari 3.1 or higher, Netscape Navigator or Communicator version 6 or higher, Adobe Acrobat Reader 6.0 or
higher and you must enable JavaScript in your browser.

You agree that by selecting the "I have read and accept" checkbox, you have reasonably demonstrated your ability to access information in the electronic form that will be used to provide you with required loan documents, notices, periodic statements and disclosures.

Your online transactions with us will be governed by the loan agreements and the terms and conditions contained therein.

Your acceptance of those separate agreements will, however, be in accordance with this Disclosure.

You consent and agree that your use of a key pad, mouse or other device to select an item, button, icon or similar act/action while using any electronic service we offer; or in accessing or making any transactions regarding any agreement, acknowledgment, consent, terms, disclosures or conditions constitutes your signature, acceptance and agreement as if actually signed by you in writing and further acknowledge and agree that the taking of any such actions by you evidence your intent to sign any such document. You also agree that no certification authority or other third party verification is necessary to the validity of your electronic signature; and that the lack of such certification or third party verification will not in any way affect the enforceability of your signature.
